Finasteride is a 5α-reductase inhibitor commonly used to treat androgenic alopecia (male pattern baldness) and benign prostatic hyperplasia (BPH). It works by reducing the body’s conversion of testosterone into dihydrotestosterone (DHT), the androgen responsible for hair follicle miniaturization and prostate enlargement. While highly effective for slowing or reversing hair loss, it does not stimulate new hair growth in completely bald areas. It’s available orally and in topical form, with topical use offering reduced systemic side effects for some users.

How It Works

Finasteride selectively blocks type II 5α-reductase, the enzyme that converts testosterone into DHT. By lowering scalp and serum DHT levels by up to 70%, it protects hair follicles from miniaturization. This action helps maintain existing hair and can regrow recently lost hair in some individuals. Lowering DHT also reduces prostate size in men with BPH. However, reducing DHT can impact sexual function, mood, and hormonal balance in a subset of users.


How to Use It

  • Dosage:

    • Hair loss: Oral 1 mg/day or topical 0.25–0.5 mg/day (formulated for scalp absorption).

    • BPH: Oral 5 mg/day.

  • Frequency: Once daily at the same time each day for stable serum levels.

  • Cycle Length: Continuous use — stopping will cause DHT levels to return to baseline within ~2 weeks, potentially restarting hair loss.

  • Stacking: Often combined with Minoxidil for enhanced regrowth, Ketoconazole shampoo for scalp DHT control, or RU58841 for targeted DHT blocking at the follicle.

  • Post-Cycle Therapy (PCT): Not required, as it is not a suppressive anabolic compound.

  • Precautions:

    • Oral form has higher risk of systemic side effects compared to topical.

    • Some users experience “post-finasteride syndrome” (persistent side effects after stopping), though this is debated in the medical community.

    • Avoid in women who are pregnant or may become pregnant — risk of birth defects in male fetuses.
